Real Health MD Breast Health Blueprint: The Smart Woman’s Guide to Breast Cancer

I hereby acknowledge that I have voluntarily enrolled in the Breast Health Blueprint: The Smart Woman’s Guide to Breast Cancer eight-week group program about breast cancer. I understand that this program is designed to provide educational information, support, and resources related to breast cancer awareness and management.

I acknowledge and agree to the following:

• I understand that the information provided in this program is for educational purposes only and is not intended to replace professional medical advice, diagnosis, or treatment. I acknowledge that I should consult with my healthcare provider regarding any medical concerns or questions I may have.
• I understand that participation in this program does not establish a doctor-patient relationship with any member of the program facilitation team or Real Health MD
• I acknowledge that Real Health MD, its employees, volunteers, and affiliates are not responsible for any decisions I make regarding my healthcare based on information provided in this program.
• I understand that participation in this program involves discussing personal experiences and sensitive topics related to breast cancer. I agree to maintain confidentiality and respect the privacy of other participants in the program.
• I acknowledge that participation in this program may involve physical or emotional discomfort. I agree to take responsibility for my own well-being and to communicate any concerns or limitations to the program facilitators.
• I understand that photographs, videos, or other media may be taken during the program for promotional or educational purposes. I consent to the use of my likeness in such materials.
• I understand that refunds are not available for this program.
